Trichoscopy of pemphigus vulgaris in remission. A tool for treatment-cessation decision making. Preliminary results

Background: One of the controversies in treatment Pemphigus Vulgaris (PV), is time cessation. Scalp involvement often most stubborn to human area and may reflect disease activity. Trichoscopy (Dermoscopy hair scalp) features have not been studied clinically “healthy-looking” skin PV patients full remission. Aims Objectives:To find out if there are any trichoscopy clinical immunologic remission, accommodate decision. Methods: The age duration 3 women with were correspondingly 62, 63, 55 5, 6, 15 years. Treatment at examination was 2,5 mg prednisone(Pr), for last 22, 5 7 months. 2nd patient also taking 1 mg/kg/d Azathioprine. Anti Desmoglein antibodies turned negative since Oct/2017, Feb/2017, Sept/2015. Contact dermoscopy hand lens X10 magnifier evaluation, used. Results: A red hue background, peripillar scales tubular casts observed by first 2 patients. Only peripilar on a small brown round 3rd patient. cessation decided only this patient, other two patients, considered as consequence outer root sheath acantholysis sign Patients’ follow up justified aforementioned decisions. Conclusions: Areas background movable cylindical (casts) around shafts be observed, even absence apparent scalp involvement, Since these drive physicians continuation, required before decision made.
